8.5.2020 – Form 10 Group Inc. is proud to announce it has been awarded a contract with the State of Florida Department of Management Services. This contract is an award for Request for Proposal 15-80101507-SA-D for Information Technology Staff Augmentation Services. This is a two-year contract that provides information technology staff augmentation services, including comprehensive management of staff provided. Over the two years our staff will need to meet ongoing performance measures put in place by the Department of Management services to insure we are consistently providing staff that meets their needs.

Form 10 Group has worked extensively for the Florida Department of Highway Safety and Motor Vehicles as well as County Tax Collector offices throughout the state. We are excited to expand this work with other state departments. Our staff consists of IT professionals who have proven themselves doing many different types of IT work.

National Veterans Small Business Engagement (NVSBE) Conference

Michael Banks and Taylor Falk of Form 10 Group attended the 9th Annual National Veterans Small Business Engagement (NVSBE) conference in Nashville Tennessee on 12/9-12/11/19. Form 10 has been a Service-Disabled Veteran Owned Small Business (SDVOSB) since 2015. During the three-day conference they met with several companies, ranging in size, capabilities and portfolio of experience.  The event also provided ready access to representatives from the Department of Veterans Affairs office.

During the past year Form 10 has been performing a variety of IT services at a large medical campus near their corporate office with the goal of expanding on this experience to gain work within the VA.  While at the conference, Form 10 met with several seasoned VA medical vendors who are working on the major IT ramp up and Electronic Health Record (EHR) modernization initiatives at the agency. EHR requires new software and hardware to be installed in the hundreds of VA Hospitals and Clinics around the country.  Any new or experienced IT service providers who would like to team up and be involved with Form 10 Group are encouraged to reach out.

Form 10 Group Recertified as a Service-Disabled Veteran-Owned Small Business

Form 10 Group is proud to announce our recertification as a Service-Disabled Veteran-Owned Small Business (SDVOSB) with the Department of Veterans Affairs. From the Veterans Affairs website: “The Vets First Verification Program affords verified firms owned and controlled by Veterans and Service-disabled Veterans the opportunity to compete for Veterans Affairs set asides.”

In order to qualify for this certification, Form 10 Group had to meet certain standards: Be a small business, be at least 51% owned and controlled by a one or more service-disabled vets, have one or more service-disabled vets manage day-to-day operations and also make long term decisions, and eligible veterans must have a service connected disability. The founders of Form 10 Group were cadets at the United States Air Force Academy which helped the company to qualify for the SDVOSB certification. Their participation in the Air Force also created a passion for service that is the foundation of Form 10 Group and guides our philosophy every day.

The Veterans Affairs Department seeks to award SDVOSBs at least 15% of its total contract dollars in 2019. In 2017 $5.1 billion in contracts was awarded to SDVOSBs which was 19.5% of the Veterans Affairs total contract accumulation. Veterans Affairs must consider SDVOSBs before they can consider other small businesses. Form 10 Group Reaches First Time SLA Milestone with FLHSMV

Tampa, Florida, August 10, 2018

Form 10 Group, Inc has received high praise for their support of the Florida Department of Highway Safety and Motor Vehicles (DHSMV) by achieving the first ever perfect score by any vendor for Service Level Agreement (SLA) requirements.  The DHSMV team was pleased to review the over 450 incidents that Form 10 responded to in July and found that every one of the tickets passed their SLA specifications.

Form 10 Group is responsible for maintaining over 24,000 pieces of equipment in 350 offices around the state of Florida for the DHSMV.  The offices span two time zones and are located in every county of the state.  The types of items maintained include enterprise class Dell servers, workstations and monitors.  Additionally, almost 6,000 Lexmark printers make up the mix of equipment that is maintained.  The SLA requirements include a 4-hour critical response time to restore service when a site is down.

Tampa, Florida, February 24, 2015— Form 10 Group receives a 96 overall customer satisfaction rating as determined by an independent survey group.   

Dun & Bradstreet conducted an audit of Form 10’s customer satisfaction using the Open Ratings Past Performance Evaluation model.  The analytics convert raw survey data into performance ratings which also take into account the standing of the company against its industry peers.

All customers surveyed rated Form 10 Group a 9 or better on the 10 point rating scale in nine categories performance was assessed.

Form 10 Group receives Hillsborough County SBE certification

Tampa, Oct 30, 2013—Form 10 Group has been certified in the Small Business Enterprise (SBE) Program through Hillsborough County. The certification opens local opportunities and increases exposure. A registered SBE can participate in competitive bidding for government contracts that receive special designations and set asides.

This county registration also formally recognizes Form 10 Group as a Hillsborough County business, although the company has been involved in the Tampa Bay community since 2009. Form 10 Group provides comprehensive IT services to Tampa area businesses and also bases its statewide deployment projects from Tampa.
